​By now, you most likely have been notified that your current plan, Excellus Blue Choice Access (PPO) is being discontinued. This means the plan will terminate on December 31st, 2024, and will not be renewed for 2025. Therefore, you must act now and enroll in a new plan for 2025 to avoid penalties.

I know this can be unsettling, but I want to reassure you that Sage Rutty is here for you. We are happy to assist you with the process of selecting a new plan.

We are conducting both virtual and in-person group Q& A discussions specifically for Excellus Blue Choice Access members to discuss your options. We will present the new 2025 plans that are available to you and will allow time for your questions as well. We highly recommend that you attend one of these seminars.

If you have already received the 2025 plan information from Excellus and have decided on a new Excellus plan for 2025, you may contact Excellus directly at 877-883-9577 and they will enroll you in the plan over the phone.
